The case is presented below; 65-year-old female, who consults the emergency unit due to pain in the left eye of 20 days of evolution, is admitted to bed rest, an MRI is performed which shows an image suggestive of orbital abscess, antibiotic treatment is started but shows no improvement, she is evaluated by the rheumatology unit finding synovitis in bilateral carpal and II III metacarpophalangeal joints, When complementary immunological tests were performed, the condition was associated to a vasculitis with positive anti-neutrophil cytoplasmic antibodies (ANCAS), treatment was started and she was evaluated by the ophthalmology department, which reported retinal detachment, cellularity in anterior and posterior chamber of the left eye, so enucleation was performed.
